I am happy to bill your insurance for your sessions. You will need to call your insurance company to ensure your plan includes massage and indicate how many massages per year you are allotted. I am credentialed with the following organizations:

• Lifewise
• PIP
• L&I

In order to bill your insurance, I require a referral form from a qualified practitioner (MD, DO, DC, or PT) with the following information:

• Written Diagnosis
• ICD-9 Code(s)
• # of Visits
• Frequency of Visits (i.e. 1x/week, 2x/month, etc)
• Pracitioner Name and NPI#
